COPE Health Solutions’ Enhanced Care Management program seeks a Community Health Worker II to join the Integrated Care Team in Los Angeles. This field-based role engages hard-to-reach members, coordinates care, and supports transitions from hospital and facilities.

Mentoring junior CHWs is included as part of the responsibilities. The CHW II conducts outreach, enrollment, and care coordination with a person-centered, trauma-informed approach, and collaborates with the Program Manager to

Qualifications

• High school graduate; associate degree preferred.
• Minimum two years’ experience as a CHW or similar role.
• Experience in street/community outreach preferred.
• Experience with ECM or Medi-Cal programs preferred.
• Training in motivational interviewing, trauma-informed care, or harm reduction preferred.
• Bilingual English/Spanish preferred.
• Computer proficiency; EMR experience preferred.
• Valid California Driver’s License with a reliable vehicle.

Responsabilités

• Identify, engage, and enroll eligible ECM members through outreach in the field.
• Develop individualized care plans with the care team and ensure member understanding.
• Coordinate appointments, transportation, and follow-up across services.
• Provide health education and support behavior change using client-centered approaches.
• Maintain documentation of interactions and care coordination in the EMR.
