Qualifications

• Bachelor’s degree in behavioral health or related field.
• 2 years of full-time behavioral health work experience.
• Bachelor’s degree in non-behavioral health field with at least 4 years’ behavioral health work experience.
• High school diploma or equivalency with 6 years’ behavioral health work experience.
• Must have Level 1 fingerprint clearance card or be eligible to receive within eight weeks of hire.
• AZ driver’s license and vehicle with state minimum insurance; able to meet Copa Health driving requirements.

Responsibilities

• Provides mentorship and training to new Case Manager hires and trainees (e.g., EMR-Next Gen, Tele-Health, Shadowing Expert).
• Collaborates with the integrated care team to engage, educate, communicate, and coordinate care with service participants and providers.
• Develops and maintains the Integrated Service Plan (ISP) within required timeframes.
• Adjusts the ISP based on progress in health and behavioral health goals.
• Maintains documentation of service delivery, assessments, coordination of care, discharge planning.
• Ensures all prescribed services in the ISP are implemented.
