Copa Health in Arizona is seeking a Case Manager Lead to advance integrated assessments and service plans, coordinating with medical, behavioral health, and dental providers to support members' recovery goals. This role focuses on comprehensive care and progress reviews within a collaborative team.

The position includes mentoring, ISP management, and maintaining detailed clinical records to ensure high-quality, person-centered services. On-site role with competitive pay and benefits.

Qualificações

• Bachelor’s degree in behavioral health field with 2 years of full-time behavioral health work experience
• Bachelor’s degree in non-behavioral health field with at least 4 years’ of full-time behavioral health work experience
• High school diploma or high school equivalency diploma and 6 years’ of full-time behavioral health work experience
• Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire
• AZ valid Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety sensitive requirements

Responsabilidades

• Mentor and train new Case Manager hires and trainees on EMR-Next Gen, Tele-Health, Shadowing Expert, etc.
• Collaborate with the integrated care team to engage, educate, communicate, and coordinate care with service participants, family, and providers
• Develop, maintain and manage the Integrated Service Plan (ISP) within required timeframes
• Adjust the ISP based on progress toward health goals
• Maintain and modify covered behavioral health and healthcare services and community resource needs
• Keep comprehensive clinical records including assessments, services, coordination of care, and discharge planning
