Job summary

UNIVERSITY OF OKLAHOM (Tulsa campus) is seeking a Senior Case Manager to support members with chronic and acute health needs through coordinated care and education. You will lead a diverse caseload, collaborate with clinicians, and mentor junior staff to ensure continuity of care and optimal outcomes.

The ideal candidate has a Bachelor’s degree in a health-related field, at least 2 years of case management/home-based services experience, and CCM certification or plan to obtain CCM within a year.

Qualifications

• Bachelor's degree in a health-related field.
• Minimum 24 months experience in home/ community-based services or case management.

Responsibilities

• Coordinate services across interdisciplinary teams to improve member outcomes.
• Mentor and onboard new team members, promoting best practices in case management.
• Advocate for members and caregivers, ensuring timely access to appropriate care.
