Qualifications

• 24 months experience in case management.
• Equivalency: Associate's degree in Nursing in lieu of Bachelor's.
• Social Work (LCSW) or Registered Nurse (RN); CCM within 1 year from hire date.

Responsibilities

• Provide leadership and mentoring to the case management team.
• Oversee care coordination, program productivity, and regulatory compliance.
