Qualifications

• Education: 2-year / Associate Degree (preferred).
• Active OK RN license.
• BLS certification.
• 1–3 years of experience as RN or in case-management related role.

Responsibilities

• Assess patient needs and coordinate delivery of patient care across the unit, social worker, and multidisciplinary team.
• Ensure patients meet admission, continued stay, and discharge criteria.
• Develop discharge plans and facilitate movement to appropriate levels of care.
• Advocate for patients with insurers and monitor treatment and financial outcomes.
• Contribute to quality improvement using trend data and educate patients and families.
