Qualifications

• 24 months experience in case management.
• Equivalency/Substitution: Will accept an Associate's degree in Nursing in lieu of the Bachelor's degree.
• Social Work (LCSW) or Registered Nurse (RN) licensure; CCM certification preferred.

Responsibilities

• Lead and mentor Case Managers to ensure exceptional care coordination and regulatory compliance.
• Contribute to program operations, quality improvement, and service delivery standards.
• Collaborate with interdisciplinary teams to strengthen workflows and documentation.
