Kennedy--donovan-Center- is seeking an Individual Home Supports Case Manager in Massachusetts to develop, coordinate, and monitor tailored supports for adults with developmental disabilities living independently or in community settings. You will provide skill training, counseling, case management, and crisis intervention while promoting person-centered planning, safety, and community inclusion.

Qualifications

• Minimum of 1 year in case management or related human services role.
• Knowledge of person-centered planning and community resources.
• Strong organizational and documentation skills.
• Ability to work independently while collaborating with teams and caregivers.
• CPR/First Aid certification or ability to obtain within designated timeframe.

Responsibilities

• Provide direct skill training, case management, counseling, and crisis intervention as needed.
• Coordinate ISPs planning and review to ensure safety and goals.
• Facilitate independence in decision-making, community involvement, and daily living skills.
• Transport individuals to appointments and activities as needed.
• Maintain accurate documentation in line with policies and regulatory requirements.
