Job summary

ICL Bronx TAP in Bronx, NY seeks a Senior Case Manager to help adults living in community apartments access medical, psychosocial, educational and vocational services, promoting independent living. You will conduct visits, develop and review treatment plans, document progress, and coordinate with providers to ensure timely supports and crisis intervention when needed.

Qualifications

• Bachelor's degree in a human services-related field.
• Minimum 1.5 years' experience providing direct services to individuals with mental disabilities.
• Excellent communication and documentation skills.

Responsibilities

• Visiting residents, developing and updating treatment plans, documenting progress and coordinating with community resources.
• Provide 1:1 or group counseling and crisis intervention as needed.
• Assist with discharge planning and integration into community supports.
