Job summary

The Senior Case Manager will support the day-to-day operations and assist the Team Leader with program guidance, aiming to ensure high-quality, person-centered care.

Minimum education is a high school diploma or GED, with preferred bachelor’s level education and related experience.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in a related field and 1 year of related experience, or 2 years of related experience.
• Ability to communicate effectively in writing.
• Read English and communicate in the program’s primary language.
• Must hold a valid driver’s license and have access to a vehicle.

Responsibilities

• Collaborate with the Team Leader to provide continuous quality improvement of individualized programs and services through the use of both internal and community resources.
• Assist team members with the development of program curriculum designed to promote individual growth and self determination.
• Develop and implement comprehensive written therapeutic service plans/individual support plans and implementation process for all assigned consumers.
• Convey changes in consumer goals to the larger internal support team.
• Maintain member records as required including timely and thorough completion of necessary program assessments, progress reports and other required documentation.
• Compile review summaries for therapeutic service plans/individual support plans.
