Catholic Charities of Acadiana seeks a Case Manager IV to provide intensive, coordinated support to our highest-acuity clients experiencing homelessness, including individuals with significant behavioral health needs and complex, co-occurring challenges.

This senior role collaborates across housing, healthcare, behavioral health, benefits, and community systems to ensure comprehensive and continuous care, while guiding Case Manager I–III and maintaining thorough documentation in compliance with

Qualifications

• Bachelor's degree required; master's preferred.
• Minimum 4 years in case management or homeless/behavioral health services.
• Experience with highest-acuity and co-occurring populations preferred.
• Knowledge of housing, benefits, and community resources.
• Strong communication and advocacy skills.

Responsibilities

• Provide intensive case management to high-acuity clients experiencing homelessness.
• Conduct comprehensive assessments and develop individualized service plans.
• Coordinate care across behavioral health, medical, housing, benefits, and community systems.
• Coordinate services with clinical providers, including discharge planning and follow-up.
• Identify gaps in services and advocate with leadership and partners.
• Maintain documentation and ensure compliance with agency and funder requirements.
