We are seeking a Case Manager IV to provide intensive, coordinated support to our highest-acuity clients experiencing homelessness, including individuals with significant behavioral health needs and complex, co-occurring challenges. This senior-level role works across housing, healthcare, behavioral health, benefits, and community systems to ensure clients receive comprehensive and coordinated care. The Case Manager IV works closely with clinical and therapeutic providers, identifies gaps in services, advocates for client needs, and supports continuity of care through crisis, hospitalization, discharge, and ongoing community services. This position also serves as a resource to other case management staff navigating complex cases. This role is ideal for an experienced, compassionate, and mission-driven professional who is committed to meeting people where they are and helping them move toward greater stability, safety, and self-sufficiency.

Key Responsibilities
  • Provide intensive case management to the highest-acuity clients experiencing homelessness, including individuals with serious mental illness, substance use disorders, and complex co-occurring needs.
  • Conduct comprehensive, strengths-based assessments and develop individualized service plans, following through on goals related to housing, benefits, income, healthcare, treatment, and long-term stability.
  • Coordinate care across behavioral health, medical, housing, benefits, legal, and other community systems to ensure clients receive comprehensive and continuous support.
  • Work closely with clinical and therapeutic providers to coordinate services, including psychiatric care, substance use treatment, inpatient placement, discharge planning, and post-hospital follow-up.
  • Identify gaps or barriers within systems of care and advocate with internal leadership and community partners to ensure client needs are appropriately addressed.
  • Provide advanced non-clinical crisis response, de-escalation, safety planning, and coordination with clinical or emergency resources when higher-level intervention is needed.
  • Conduct frequent field-based visits and participate in case conferences and multidisciplinary team meetings to support complex client needs.
  • Serve as a subject-matter resource to Case Manager I, II, and III staff navigating complex, high-acuity, or behavioral-health-intersecting cases.
  • Maintain accurate, timely documentation and ensure compliance with agency, program, and funder requirements.
  • Model professionalism, compassion, ethical practice, and the values of Catholic Charities of Acadiana in every interaction.
  • All other duties as assigned.
Job Qualifications
  • Education: Bachelor's Degree in Social Work, Psychology, Human Services, or related field required; Master's Degree in Social Work, Counseling, Psychology, or a related human services field preferred.
  • Experience: Minimum 4 years experience in case management, homeless services, or behavioral health systems required; direct experience with highest-acuity and co-occurring populations strongly preferred.
  • Deep working knowledge of behavioral health conditions, treatment modalities, and levels of care, with a demonstrated ability to coordinate clinical services.
  • Demonstrated experience collaborating with clinical and behavioral health providers as part of an interdisciplinary care team.
  • Strong knowledge of housing systems, benefits systems, and community resources, with the ability to identify and clearly articulate systems-level service gaps to partners.
  • Excellent interpersonal, communication, and advocacy skills with a compassionate, nonjudgmental approach.
  • Proficiency with electronic case management systems and standard office software.
  • Ability to manage high-acuity, unpredictable situations with professionalism and calm.
  • Commitment to continuous learning, professional growth, and ethical excellence.
