Job summary

Your Health Organization in Rock Hill is seeking aSenior Solutions Advisor to help patients and families navigate aging, chronic illness, and care transitions. The role travels to patients’ homes and facilities in the Rock Hill/York area, promoting patient-centered solutions and connecting individuals to healthcare and caregiving options.

This full-time, salary-based position runs Monday–Friday, 8:00 AM–5:00 PM, with travel between sites.

Qualifications

• Strong interpersonal and communication skills.
• Self-starter with proven time management and follow-through.
• Must be a licensed driver with reliable transportation.
• Ability to read and communicate effectively.

Responsibilities

• Meet with patients and families to assess long-term care needs.
• Provide guidance on living arrangements and available services.
• Serve as regional contact for onboarding new patients.
• Maintain knowledge of local senior care resources.
• Coordinate access across resources for seamless care.
