Job summary

BoldAge PACE is seeking a dedicated Social Worker (MSW) to provide direct social work and care management for program participants in the Florida area. You will screen participants, manage cases, offer counseling, make referrals, and coordinate services with families and community agencies.

The role emphasizes advocacy, holistic well-being, and collaboration with an interdisciplinary team to promote independence and quality of life for older adults living at home or in community settings.

Qualifications

• Master's degree in Social Work from an accredited CSWE-program.
• Current License in the required state.
• Two years of experience on an interdisciplinary team in healthcare settings preferred.
• One year of experience with frail or elderly population; training available if not.

Responsibilities

• Participate in initial assessments, care planning, and periodic reassessments.
• Document participant changes and updates in the medical record.
• Collaborate with Interdisciplinary Team (IDT) members regarding participant status and care plans.
• Coordinate 24-hour care delivery and provide individual and family counseling.
• Refer participants and families to appropriate community resources and advocate for needed services.
• Participate in IDT meetings and contribute to care planning.
• Conduct home visits to evaluate and support participant needs.
