Qualifications

Responsibilities

• Participates in interdisciplinary team assessments, care planning and reevaluations.
• Maintains current written case management records and reassessments.
• Refers participants and families to community agencies and advocates for participants.
• Collaborates to build a cohesive team and supports staff relationships.
• Advises on social, emotional, and cultural factors in health care and community resources.
• Participates in program and policy development for Florida PACE Centers.
• Attends meetings as needed.
