Job summary

BoldAge PACE is seeking a Social Worker (MSW) in Owensboro, KY for a full-time role providing direct social work and care management services to participants in the PACE program. Responsibilities include screening, case management, counseling, and referrals while coordinating care with the interdisciplinary team to promote independence and well‑being.

Qualifications

• Master’s degree in social work from an accredited school of social work.
• Certification preferred (ACSW or NASW-recognized).
• Current license in the required state.

Responsibilities

• Participate in initial assessments and care planning for participants.
• Document changes in medical records accurately and timely.
• Collaborate with the Interdisciplinary Team (IDT) to coordinate care.
• Maintain up-to-date case management records and reassessments.
• Provide individual and family counseling and coordinate 24-hour care delivery.
• Refer participants to community resources and advocate for access to services.

Education

Master’s degree in social work

ACSW certification or NASW-recognized certification preferred
