Job summary

BoldAge PACE is hiring a Social Worker (MSW) to provide direct social work and care management to participants in our PACE program, helping them live independently with dignity. You will screen, plan, counsel, and coordinate services with families and the interdisciplinary team.

The role emphasizes advocacy, coordination of community resources, and ongoing documentation to support participant well-being and program goals in a senior care setting.

Qualifications

• MSW from an accredited school of social work.
• Licensed to practice in the required state.
• Experience on an interdisciplinary team preferred.

Responsibilities

• Provide direct social work and care management services to participants.
• Screen participants, plan care, and coordinate services with caregivers.
• Counsel individuals and families; refer to community resources as needed.
• Maintain accurate documentation in medical records and IDT meetings.
• Conduct home visits and support care planning and updates.
