Qualifications

• Bachelor's Degree in Social Work, Human Services, Psychology, or a related field.
• SNF, rehabilitation, long-term care, or healthcare experience preferred.
• Previous leadership or supervisory experience strongly preferred.
• Strong knowledge of discharge planning, care coordination, and resident advocacy.
• Excellent interpersonal, communication, and organizational skills.
• Compassionate, dependable, and committed to providing exceptional service to seniors.

Responsibilities

• Direct and oversee all aspects of the Social Services Department.
• Develop, coordinate, and implement resident-centered social service programs.
• Assist residents and families with admissions, care planning, transitions, and discharge planning.
• Advocate for resident rights and ensure compliance with state and federal regulations.
• Collaborate with physicians, nursing staff, therapy teams, and community agencies to support positive resident outcomes.
• Provide counseling, emotional support, and crisis intervention when needed.
• Build strong relationships with residents, families, and community resources.
• Maintain accurate documentation and participate in interdisciplinary care team meetings.

Education
