Job summary

Continuum Healthcare Inc. seeks a Social Services Director/Designee and Admissions Coordinator to plan and direct the facility's social services. You will oversee assessments, care plans, and discharge planning, ensuring regulatory compliance and resident-focused care.

You will lead policy development, quality assurance, and staff supervision, coordinating with administration and clinical teams to meet residents' medical and emotional needs with compassion and professionalism.

Qualifications

• Master's in social work preferred, or a relevant advanced degree.
• Minimum Bachelor's degree in social work, sociology, education, psychology, or related field, or a state license.
• Minimum 2+ years supervisory experience in a hospital, nursing facility, or similar setting.
• Ability to read, write, speak, and understand English; strong leadership and coordination skills.

Responsibilities

• Plan, develop, organize, implement, and direct social services programs.
• Discharge planning, resident assessments, and development of care plans.
• Coordinate budgets, policies, and quality assurance for the Social Services Dept.
• Maintain HIPAA compliance and liaison with residents, families, and staff.
• Interact with administration, medical and nursing staff to integrate social services.
• Review and update department policies, ensure regulatory compliance.
