Qualifications

• Bachelor’s Degree in social work, gerontology, or related social science preferred, else five years of relevant experience
• Experience networking with senior citizens and arranging elderly support services
• Ability to relate well with residents and professionals

Responsibilities

• Establish and maintain relationships with local, state, and federal agencies and local service providers.
• Develop and maintain a directory of community-based providers for referrals.
• Read lease and property rules; inform Community Manager of any lease violations.
• Identify at-risk or frail residents and refer to appropriate services.
• Provide basic case management: assess ADLs, arrange services, liaise with caregivers, monitor quality, reassess needs.
• Maintain resident files with progress notes, reports, and case resolution.
• Coordinate wellness screenings and health-related programs at the property.
• Collaborate with Community Manager to ensure continued residency or move to higher care when needed.
• Submit monthly activity reports to Coordinator for Service Programs.
