Qualifications

• This position is ideal for candidates with experience in medical scheduling, client coordination, case coordination, or client intake.
• 1+ year of experience in a home care agency, healthcare office, scheduling care, or client coordination position.
• Strong organizational and multitasking abilities.
• Excellent phone, communication, and customer service skills.
• Comfortable working with computers, scheduling software, Microsoft Office, and data entry.
• Ability to prioritize tasks and problem solving in a fast-paced home care environment.

Responsibilities

• We are seeking an organized, detail-oriented Client Care/Scheduling/Staffing Coordinator to help manage client intake, scheduling, and ongoing care coordination
• Coordinate new client referrals and admissions
• Schedule and assign Care Professionals/Caregivers, Home Health Aides, and Personal Care Aides based on client needs, availability, and experience
• Serve as the primary point of contact for clients, caregivers, and client contacts
• Manage client intake, onboarding, and follow-up communications
• Track visits, authorizations, documentation, and scheduling changes within the scheduling software system
• Maintain accurate client records and data entry
• Handle inbound calls, client inquiries, and scheduling requests
• Communicate with care professionals/caregivers regarding schedules, documentation, and client status updates
• Support day-to-day operations to ensure timely and efficient client care delivery
