About This Role

The Senior Care Manager (RN) is responsible for assessing, planning, and implementing complex care management activities to ensure quality, cost-effective healthcare outcomes for members. This role is ideal for experienced nurses who are skilled in developing personalized care plans and collaborating with healthcare providers.

Highlights

• Pay: $75,300 - $135,400/year
• Location: Remote, MI
• Schedule: Full Time, Day shift
• Travel: Up to 75% local travel to support members in Wayne and Macomb counties

What You'll Do

• Develop and continuously assess ongoing care plans/service plans.
• Collaborate with providers to identify resources needed to address member's unmet needs.
• Coordinate and manage care between the member and care provider team.
• Monitor care plans and member status, revising as necessary.
• Identify problems/barriers for care management and appropriate interventions.
• Review member data to identify trends and improve quality care.
• Develop appropriate care plans/service plans based on referrals and assessments.
• Perform outreach to assess member needs and collaborate with resources.
• Collect, document, and maintain all member information for compliance.
• Provide education to members and their families on healthcare options and benefits.

Requirements

Education:

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required

Experience:

• 4 – 6 years of related experience required

Licenses & Certifications:

• RN – Registered Nurse – State Licensure and/or Compact State Licensure required
