Volunteers of America - National Services invites a Registered Nurse (RN) to join the Senior CommUnity Care of Northern Kentucky PACE in Florence. The role delivers general and skilled nursing care to participants, executes plans of care, and coordinates with the care team to maintain safety and independence in the community.

Qualified candidates hold an active KY RN license, CPR certification, and at least one year of elderly care experience, with access to reliable transportation.

Qualifikationen

• Current licensure in the state of Kentucky as a Registered Nurse.
• Must have a valid driver’s license, proof of insurance and have means of transportation.
• Medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
• Current CPR Certification.
• A minimum of one-year experience working with frail or elderly population.

Aufgaben

• Participates in Interdisciplinary Team meetings and communicates participant changes.
• Develops and implements planned care to meet participant needs.
• Provides skilled nursing care in clinic and during home visits.
• Triages participants to determine priority of care and assists in examination and treatment.
• Maintains participant records and ensures documentation aligns with policy.
