Job summary

Senior Community Care of Northern Kentucky in Florence, KY is seeking a Registered Nurse to provide general and skilled nursing care to participants enrolled in the PACE program. You will implement care plans, coordinate with the interdisciplinary team, and educate participants, caregivers, and families.

The RN role requires Kentucky licensure, CPR certification, a valid driver's license, and at least 1 year of experience with frail or elderly populations.

Qualifications

• Current licensure in the state of Kentucky as a Registered Nurse.
• Must have a valid driver’s license and transportation.
• Current CPR Certification.
• Minimum 1 year experience with frail or elderly population.

Responsibilities

• Provide general and skilled nursing care to participants per plan of care.
• Implement physician orders and evaluate participant conditions.
• Coordinate care with the Interdisciplinary Team for 24-hour care delivery.
• Conduct initial nursing assessments and semi-annual re-assessments.
• Perform care and education for participants and families.
• Participate in on-call rotation for after-hours and weekend care.
• Maintain participant records and documentation per policy.
• Performs care coordination with outside providers and facilities.
