Job summary

Senior CommUnity Care of Northern Kentucky, part of Volunteers of America National Services, is seeking a Registered Nurse to join our PACE team. You will provide general and skilled nursing care, develop and implement care plans, and educate families and participants to promote safe, community-based living.

The role emphasizes collaboration within an interdisciplinary team, on-call coverage, and adherence to comprehensive care standards across home and clinic settings.

Qualifications

• Current KY RN licensure is required.
• Valid driver’s license with reliable transportation.
• CPR certification is required.
• Minimum 1 year experience with frail or elderly populations.

Responsibilities

• Coordinate care with the Interdisciplinary Team to ensure 24-hour care delivery.
• Develop and implement plans of care to help participants remain safely at home.
• Perform initial and ongoing nursing assessments for new enrollees and adjust plans as needed.
• Provide skilled nursing care per physician orders and document conditions and responses to treatments.
• Participate in on-call rotations and respond to after-hours needs.
• Maintain accurate records and ensure compliance with policies and OSHA guidelines.
