Volunteers of America National Services’ Senior CommUnity Care of Northern Kentucky PACE is seeking a Registered Nurse (RN) in Florence, KY. You will provide general and skilled nursing care, coordinate with the interdisciplinary team, and educate participants and families while ensuring safe, effective care in the community.

The role offers a supportive team, opportunities for growth, and a mission-driven work environment focused on seniors and care coordination within the PACE program.

Qualifikationen

• Current licensure in the state of Kentucky as a Registered Nurse.
• Must have a valid driver’s license, proof of insurance and transportation.
• Medical clearance for communicable diseases and up-to-date immunizations.
• Current CPR Certification.
• Minimum of one year experience with frail or elderly population.

Aufgaben

• Participates in Interdisciplinary Team meetings and coordinates care.
• Develops and implements a care plan to help participants remain at home.
• Conducts initial nursing assessments for new enrollees.
• Performs general and skilled nursing care and follows physician orders.
• Provides nursing education to participants and families.
• Manages participant records and documentation per policy.
• Ensures compliance with laws, safety guidelines and organizational standards.
