Pro Rehab is hiring a Physical Therapist to join Maple Lawn Senior Care in Fulda, MN. The PT will evaluate and treat patients to improve mobility, relieve pain, and restore function with individualized plans.

Responsibilities include assessment, program development, hands-on therapy, patient education, progress monitoring, and collaboration with healthcare teams. Flexible start/end times and a competitive wage are offered.

Qualifications

• Must be a graduate from an Accredited Physical Therapy Program and have a bachelor's degree.
• Must possess a current MN Physical Therapist License.
• Must possess strong knowledge of state, federal, and local Home Health regulations.
• Must be able to perform the essential functions of the job, with or without reasonable accommodations.
• Must be able to communicate in English, both verbally and in writing.

Responsibilities

• Assess & Diagnose: Evaluate patients' physical conditions and movement abilities to develop personalized treatment plans.
• Develop & Implement Treatment Plans: Create rehabilitation programs to improve functional deficits.
• Provide Hands-on Therapy: Perform manual therapy, therapeutic exercises, and modalities as needed.
• Educate & Support Patients: Provide education to patients and caregivers on exercises and injury prevention.
• Monitor Progress & Adjust Plans: Track outcomes and modify plans as needed.
• Collaborate with Healthcare Teams: Work with doctors, nurses, and other therapists to ensure comprehensive care.
