Job summary

Cassia Community seeks a dedicated Physical Therapist to join the team at Maple Lawn Senior Care in Fulda, MN. The PT will evaluate, treat, and educate patients to improve mobility and reduce pain, while building a caseload in a rural setting with supportive teams.

Position is full-time with flexible start times; wage range is $45–$55 per hour, plus a $7,500 bonus paid over 18 months and tuition reimbursement.

Qualifications

• Must be a graduate from an Accredited Physical Therapy Program and have a bachelor’s degree.
• Must possess a current accredited MN Physical Therapist License.
• Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care.
• Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.

Responsibilities

• Assess and Diagnose: Evaluate patients' physical conditions, movement abilities, and medical history to develop personalized treatment plans.
• Develop and Implement Treatment Plans: Create and execute rehabilitation programs to improve functional deficits.
• Provide Hands-on Therapy: Perform manual therapy, therapeutic exercises, and use modalities such as ultrasound and electrical stimulation as needed.
• Educate and Support Patients: Provide patient, caregiver, staff education on exercises, injury prevention, and pain management techniques.
• Monitor Progress and Adjust Plans: Track patient outcomes, document progress, and modify treatment plans as needed.
• Collaborate with Healthcare Teams: Work with doctors, nurses, and other therapists to ensure comprehensive patient care.

We are looking for a Physical Therapist to join the team at Maple Lawn Senior Care, in Fulda, MN! Maple Lawn Senior Care is in a rural environment that offers a calmer atmosphere and less stress, allowing you more opportunities to build a full caseload quickly.

As our Physical Therapist (PT), you will evaluate and treat patients to improve mobility, restore physical function, and reduce pain through individualized treatment plans. Your responsibilities will include assessing patients’ physical conditions and movement abilities, developing and implementing rehabilitation programs, providing hands‑on therapy and therapeutic exercises, educating patients and caregivers on exercises and injury prevention, monitoring progress, and collaborating with healthcare teams to support quality patient care. To be successful, you must have strong knowledge of physical therapy techniques, excellent assessment and communication skills, and the ability to provide compassionate, patient‑centered care.

We offer a competitive wage, benefits, and up to a $7,500 bonus paid out over 18 months or tuition reimbursement. Physical Therapist Qualifications:
