Qualifications

• Graduate from an Accredited Physical Therapy Program and hold a Bachelor’s degree.
• Must possess a current MN Physical Therapist license.
• Must understand state, federal, and local Home Health regulations.

Responsibilities

• Assess and diagnose patients to develop personalized treatment plans.
• Develop and implement rehabilitation programs to improve mobility and function.
• Provide hands-on therapy and therapeutic exercises.
• Educate patients and caregivers on exercises and injury prevention.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with doctors, nurses, and other therapists to ensure quality patient care.
