The ideal candidate is licensed in Minnesota, graduates from an accredited PT program, and can build a full caseload in a rural setting. New grads are welcome, with opportunities to work with a collaborative healthcare team and provide patient-centered rehabilitation services.

Qualifications

• Graduate from an Accredited Physical Therapy Program and licensed in MN.
• Bachelor's degree in Physical Therapy.
• Knowledge of Home Health regulations (state, federal, local).
• Proficient English communication.

Responsibilities

• Assess & Diagnose: Evaluate patients' conditions and history to plan treatment.
• Develop & Implement Treatment Plans to improve function.
• Provide Hands-on Therapy via manual therapy and modalities.
• Educate & Support Patients and caregivers on exercises and prevention.
• Monitor Progress & Adjust Plans based on outcomes.
• Collaborate with doctors, nurses, and other therapists for comprehensive care.
