Job summary

Housecall Providers in Salem seeks an experienced Physician Assistant to deliver primary care in patient residences, focusing on chronic disease management and palliative care. You will collaborate with physicians to diagnose, treat, and follow up, with emphasis on goal-based care and continuity of service.

The role involves travel within Marion/Polk counties, after-hours coverage, and maintaining comprehensive medical records. Oregon license, NCCPA, DEA, and CPR are required.

Qualifications

• Master’s level PA program and licensure in Oregon.
• Current NCCPA certification and DEA license.
• Minimum 2 years clinical experience in primary care, geriatrics, or palliative care.
• Valid driver’s license and reliable transportation.
• CPR certification at hire or within 6 months.

Responsibilities

• Provide diagnosis, treatment, consultation, and follow-up care with a licensed physician.
• Perform comprehensive health assessments and make referrals to social/medical services.
• Develop proactive patient-centered care plans with the care team.
• Order tests and medications with follow-up and reassessment; perform some procedures as appropriate.
• Approve medication refills and home health orders; respond to caregiver requests.
• Deliver high-quality palliative care focused on patient and family goals.
• Maintain medical records per agency protocols; participate in QA activities.
