Qualifications

• Bachelor's degree or higher in Physical Therapy or related field.
• Active state license required.
• Minimum 2+ years of experience in a skilled nursing facility.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop plan of care.
• Provide rehabilitative, skilled and medically necessary treatment.
• Administer procedures as part of the rehabilitation plan (ADL retraining, exercises, modalities).
• Document treatment minutes and outcomes in the electronic medical record.
• Coordinate with supervisor and health team members regarding patient progress and discharge planning.
