Willow Point Rehabilitation & Nursing Center in Kansas City, KS seeks a licensed Physical Therapist for a full-time role in our in-house rehab program. You’ll screen and treat patients with cognitive and ADL impairments, develop plans of care, and collaborate with the care team to promote recovery.

Responsabilités

• Screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop plan of care.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled, and medically necessary treatment per regulatory requirements.
• Administer procedures as part of rehabilitation plan (ADL retraining, exercises, modalities, and assistive devices).
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate outcomes and modify services; refer as needed.
• Provide counseling to patients and families related to physical disorders.
• Communicate progress, barriers, and plans with supervisor and care team.
• Provide comprehensive discharge summaries.
