Job summary

BoldAge PACE in Fresno, CA is seeking a Physical Therapist to plan and implement physical therapy services for PACE participants, including assessment, treatment, and teaching families to maintain support in the community. The role requires an advanced degree in PT, CA licensure, and 2 years on an IDT; bilingual Hmong preferred.

You will work in center, clinic, or home settings, collaborating with the care team.

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state of California.
• Two years of interdisciplinary team experience preferred.

Responsibilities

• Plans, organizes, and implements Physical Therapy services for PACE participants.
• Performs in-person assessments at enrollment and ongoing as determined by the IDT.
• Assess needs for adaptive equipment and home safety modifications.
• Develops and adjusts physical therapy treatment plans to meet goals.
• Documents evaluation, plan, and treatment accurately in medical records.
• Educates participants and families in procedures and home programs.
• Trains PACE staff in body mechanics and safety for ADLs.
• Collaborates with IDT to develop comprehensive care plans.
