Job summary

Legacy Healthcare Services, Inc. is seeking a Physical Therapist to join our Ivy Creek team. This role offers full-time or part-time hours with a Monday–Friday schedule, no weekend or holiday requirements, and opportunities to grow within a not-for-profit organization dedicated to senior care.

You will deliver individualized sessions in a rehabilitation setting with a focus on patient-centered care. You will plan, implement, and adjust treatment plans, collaborate with a multidisciplinary team,

Qualifications

• Degree in area of discipline.
• Current Physical Therapy license in state of practice, or ability to obtain prior to starting employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to patients under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for patients.
• Treat residents one-on-one with individualized attention through private sessions.
• Individualize treatment sessions based on patient needs.
