Legacy Healthcare Services, Inc. is hiring a Physical Therapist to provide individualized rehabilitative care to senior residents in our communities. You will plan and execute treatment programs, coordinate with the care team, and participate in clinical conferences to ensure optimal patient outcomes.

The role emphasizes one-on-one sessions, functional-based therapy, and can include licensure reimbursement and ongoing CEUs. A Monday–Friday schedule with no weekends supports work-life balance.

Qualifications

• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment.
• Degree in area of discipline.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one, not in a group setting, so each patient gets individualized attention through private treatment sessions with a functional based treatment approach.
• You have the ability to individualize treatment sessions!
