Job summary

Legacy Healthcare Services, Inc. seeks a dedicated Physical Therapist to deliver rehab services in senior living communities.

You will plan and implement individualized treatment plans, work with a coordinated team, and participate in clinical conferences to optimize patient outcomes. This role emphasizes one-on-one treatment sessions, a Monday–Friday schedule with no weekend duties, and opportunities for ongoing CEUs and career growth within a not-for-profit organization that values

Qualifications

• Degree in the relevant physical therapy discipline.
• Current Physical Therapy license in state of practice, or ability to obtain prior to starting.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one with individualized attention through private treatment sessions.
