Job summary

Sage Health is seeking a Primary Care Physician to lead a team-based, value-oriented care model focused on seniors. The PCP works with clinicians, pharmacists, and care coordinators to deliver coordinated primary care within a concierge framework and Medicare Advantage partnerships.

The role emphasizes patient-facing time, high-quality outcomes, and deep community ties, with a base salary plus bonus and comprehensive benefits.

Qualifications

• Graduate of accredited MD or DO program.
• Current and unrestricted medical license; eligible to obtain licenses in other states as needed.
• Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
• Active and unrestricted DEA license.
• Medicaid Provider Number.
• Demonstrate strong interpersonal relationships and communication with colleagues and patients.
• Fully engaged in team-based care and value-based practice.
• Willingness to learn and adapt to value-based care settings.
• Basic computer skills and EMR familiarity.
• TB screening if selected.

Responsibilities

• Evaluates and treats center patients per standard of care.
• Coordinate patient services including referrals, hospital/SNF coordination, equipment and home health care.
• Act as a key medical resource within the care team during daily huddles.
• Support Lead Physician and Center Administrator in fostering cooperation and professionalism.
• Complete medical records daily with a coder to optimize documentation.
• Follow policies defined by Clinical Leadership.
• Meet with Lead Physician to review quality of care and outcomes data.
