Job summary

Sage Health in Mobile, AL, seeks an experienced Primary Care Physician to lead a team-based care model delivering concierge primary care for seniors. Our value-based approach emphasizes high-quality outcomes and coordinated care within Medicare Advantage networks.

Qualifications

• Graduate of an MD or DO program from an accredited university.
• Current medical license or willingness to obtain licenses in the region.
• Board-certification or eligibility in Family/Internal/Geriatric Medicine preferred.
• Active DEA license and Medicaid provider number.
• Strong interpersonal skills and teamwork in a value-based care setting.
• Proficient with basic computer tools including email and EMR.
• Willing to be screened for TB.

Responsibilities

• Evaluates and treats center patients in accordance with standards of care.
• Coordinate patient services including referrals, hospital/SNF coordination, and home health care.
• Act as a medical expert within the care team during daily huddles.
• Lead physician and center administrator in maintaining professional, collaborative practice.
• Complete medical records daily with a quality-based coder to optimize documentation.
• Follow policy and protocol defined by Clinical Leadership.
• Meet with Lead Physician to review quality of care and outcome data.
• Balance direct patient care with charting and meetings.
