Job summary

Sage Health in Mobile, AL, is seeking a Primary Care Physician to lead a team-based care model that delivers concierge-style primary care. You will collaborate with physicians, advanced practice providers, pharmacists, and care coordinators to provide high-quality care within a value-driven framework.

The role focuses on spending more time with patients, coordinating services, and improving outcomes for seniors through analytics and coordinated care.

Qualifications

• Graduate of an MD or DO program from an accredited university.
• Current and unrestricted medical license; eligible to obtain licenses in other states in the region.
• Board Certification or eligible in Family Medicine, Internal Medicine or Geriatric Medicine preferred.
• Active and unrestricted DEA license.
• Medicaid Provider Number.
• Demonstrates high level of interpersonal relationships and communication with colleagues/patients.
• Fully engaged in team-based care model.
• Willingness to learn/adapt to value-based care setting.
• TB screening will be required if selected.

Responsibilities

• Evaluates and treats center patients in accordance with standards of care.
• Coordinate patient services including referrals, hospital/SNF coordination, and home health care.
• Act as a key medical expert with the care team during daily huddles.
• Lead Physician and Center Administrator by maintaining professional, approachable demeanor.
• Complete medical records daily with a quality-based coder to optimize documentation.
• Follow policies and protocols defined by Clinical Leadership.
• Meet with Lead Physician about quality of care, outcome data, and medical records issues.
• Focus on direct patient care including patient-facing time and related administrative tasks.
