Job summary

Legacy Home Care is seeking a Community Liaison for the Greater Twin Cities Metro Area to build referral relationships with hospitals, senior living communities, and care teams. You will identify patients discharging home who need support and position Legacy Home Care as the trusted provider families turn to for private pay services.

Qualifications

• Bachelor’s degree in Business Administration or related field is required.

Responsibilities

• Build referral relationships with social workers, case managers, discharge planners, senior living leaders, and community partners.
• Identify families who may need private pay care after hospitalization, rehab, surgery, or functional decline.
• Educate referral sources and families on private duty care options and Legacy’s support for safe transitions home.
• Create a repeatable referral process through outreach, follow-up, and reliable service.
• Attend networking events, community meetings, and partner visits to increase brand visibility.
• Manage long-cycle opportunities where payoff may come weeks or months after first contact.
• Track activity, lead sources, referral outcomes, and client conversions.
