Job summary

SYNERGY HomeCare is seeking a Community Liaison in the Los Angeles area to grow our non-medical home care referral network. This field-based role will focus on relationships with assisted living facilities, skilled nursing centers, hospitals, and physician offices to generate qualified opportunities.

Qualifications

• Experience in home care or healthcare sales preferred.
• Outgoing, confident and comfortable meeting new people.
• Motivated by goals, commissions, and performance incentives.
• Able to manage a field-based schedule with consistent follow-up.

Responsibilities

• Visit local senior living and healthcare organizations regularly.
• Build relationships with social workers, discharge planners, case managers, administrators, nurses, and care coordinators.
• Introduce SYNERGY HomeCare and explain non-medical services.
• Identify families and seniors who may need home care assistance.
• Follow up with referral partners and prospective clients.
• Attend networking and senior-focused events and develop new referrals.
• Track visits, contacts, referrals, and follow-up activities.
• Coordinate with office team to assist referred families.
• Represent SYNERGY HomeCare professionally in the community.
