Job summary

Curana Health is seeking an APP (Nurse Practitioner or Physician Assistant) to provide holistic, compassionate care to Medicare Advantage plan members in collaboration with physicians, Lead Nurse Practitioner, and care teams. The APP will diagnose, manage, and prevent acute and chronic conditions, while promoting wellness within a collaborative practice.

Qualifications

• Valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Performs in-depth health risk assessments on all assigned plan members.
• Works collaboratively with physicians, Care Coordinators, facility staff, family members, and other healthcare personnel.
• Identify quality improvement activities aligned with policies and protocols.
• Reviews and interprets medical, surgical and social histories of members.
• Performs appropriate screening procedures for members.
• Interprets lab and diagnostic data and recommends treatments or actions.
• Prescribes or performs treatments including medications, injections, wellness counseling, and testing.
• Complies with OSHA and State Board of Nursing requirements.
• Participates in mandatory compliance and training programs.
