Job summary

Curana Health, Inc. seeks an experienced Advanced Practice Provider (APP) to deliver holistic care to Medicare Advantage plan members in collaboration with physicians, care coordinators, and RNs.

The APP role emphasizes independent nursing functions within a physician-guided framework, with a focus on health risk assessments, treatment planning, and quality improvement while maintaining HIPAA compliance and strong patient rapport.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Master's Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Performs in-depth health risk assessments on all assigned plan members.
• Works collaboratively with physicians, Care Coordinators, facility staff, family members and other medical personnel to coordinate care.
• Responsible for identifying overall quality improvement activities through assessing and planning health care individualized to the member and consistent with policies.
• Reviews and interprets the medical, surgical and social history of members; performs screenings and interprets data.
• Participates in prescribing or managing treatments and wellness counseling as appropriate; adheres to HIPAA rules.
• Maintains compliance with OSHA and State Board of Nursing; completes mandatory training and compliance programs.
• Refers members to PCP and Care Management programs as necessary; collaborates with facility leadership to achieve member care goals.
