Job summary

Curana Health is seeking a Nurse Practitioner for collaborative, on-site care of Medicare Advantage plan members across senior living facilities. You will work with physicians, care coordinators, RNs and other clinicians to diagnose, manage, and improve member health outcomes.

The APP practice includes independent nursing functions within scope of practice and requires a state NP license, a Master's in Nursing, and strong communication skills. Travel may be required.

Qualifications

• Valid NP license in the operating state
• Master's degree in nursing or PA studies
• Strong leadership, interpersonal, and problem-solving skills
• Excellent written and oral communication

Responsibilities

• Performs in-depth health risk assessments for plan members
• Collaborates with physicians, care coordinators, and facility staff
• Identifies opportunities for quality improvement in healthcare delivery
• Reviews medical histories and interprets lab results
• Recommends treatments and coordinates follow-up actions
• Prescribes or administers medications and wellness counseling
• Complies with HIPAA and all relevant laws and policies
• Supports care management programs and client meetings
