Job summary

Greater Good Health is hiring a Nurse Practitioner to deliver care to senior Medicare patients in a hybrid setting, balancing in-clinic dialysis center work with in-home visits. The role emphasizes meaningful patient engagement, coordinated care, and expert documentation within HIPAA guidelines.

The NP will work with an integrated care team, provide comprehensive assessments, and develop individualized care plans while flexing across regions to meet patient and partner needs.

Qualifications

• NP certification from ANCC or AANP is required.
• 2+ years as NP in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• Current DEA licensure/prescriptive authority post-hire and CPR/BLS.

Responsibilities

• Support highly complex patients and navigate the healthcare system.
• Provide patient education, care plans, and clinical guidance to an integrated care team.
• Coordinate care with dialysis and nephrology partners and other providers.
