Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification of AANP or ANCC
• Active DEA license
• 2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult or family practice setting
• Passionate about providing healthcare to the senior and low-income populations

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Develop treatment plans and clearly communicate them to residents and other providers.
• Order and interpret lab and diagnostic tests and adjust treatment accordingly.
• Maintain compliant documentation in the EMR.
• Collaborate with local and regional operations and clinical leadership to meet goals.
• Support value-based care initiatives.
• Provide an excellent customer experience to foster satisfaction and retention.
• Collaborate with PCP and interdisciplinary care team at the facility.
• Coordinate care with telemedicine providers and other specialists.
• Complete regulatory monthly, quarterly, and annual visits as needed.
• Participate in family conferences and provide advanced care planning and education.
• Address pharmacy recommendations and communications.
• Be present in assigned facilities during work hours to ensure face-to-face coverage.
