Job summary

Personalized Health Partners (PHP) is seeking a Nurse Practitioner (Adult and/or Gerontology) to provide high-quality primary care for residents across settings, including telehealth. The NP will diagnose, treat, and manage chronic and acute conditions, collaborate with PCPs and interdisciplinary teams, and participate in family conferences to develop care plans.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification of AANP or ANCC
• Active, unrestricted state license
• Active DEA license
• US work authorization
• 2+ years clinical experience as a Nurse Practitioner in geriatric, adult or family practice setting
• Be open to professional development and attending team meetings

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education.
• Develop treatment plans and discuss them with residents and providers.
• Order and interpret lab and diagnostic testing and adjust treatment plans accordingly.
• Maintain documentation within the EMR.
• Collaborate with operations and clinical leadership to achieve goals.
• Support value-based care initiatives.
• Provide an excellent customer experience to foster satisfaction and retention.
• Collaborate with PCP and interdisciplinary care team at facility.
• Coordinate care with other providers and telemedicine.
• Complete regulatory required visits.
• Participate in family conferences and advanced care planning.
