Eventus WholeHealth seeks an Advanced Practice Clinician (NP or PA) to deliver comprehensive primary care in skilled nursing and senior living communities. You will perform routine rounding, manage chronic diseases, and coordinate care with interdisciplinary teams and facility partners.

The role emphasizes preventive care, high-quality outcomes, and a better patient, family, and staff experience, with a collaborative, growth-oriented work environment.

Qualifications

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams
