Job summary

UnitedHealth Group is seeking an Advanced Practice Clinician (NP/PA) for the Senior Community Care program in a fast-growing, patient-centered environment. You will conduct comprehensive assessments, support short-term sub-acute rehabilitation, and coordinate care across nursing homes, senior housing and home settings.

Role requires state licensure as an NP or PA, DEA prescriptive authority, and ability to travel up to 100% within a designated area.

Qualifications

• Active NP or PA licensure in the practicing state or clearly obtainable by start date.
• Active and unrestricted DEA licensure/prescriptive authority or ability to obtain post-hire.
• Reliable transportation and driver's license required.
• Experience with electronic medical records is preferred.

Responsibilities

• Conduct comprehensive assessments including history, vitals, exam, and medication reconciliation.
• Provide care coordination for high-risk health plan members in various care settings.
• Document accurately and completely with ICD-10 coding.
• Collaborate with interdisciplinary care teams and support advanced illness planning.
